  • NOTE: If you plan on getting married within the next 30 days, we cannot guarantee issuance and delivery of the license in time for the ceremony. It would be in your best interest to make an appointment online and come into one of our locations to be issued the license the same day.
  • NOTE: You will need one of the following fully supported browsers in order to use this process and complete the forms: Microsoft Internet Explorer 11, Microsoft Edge (EdgeHTML 16 and greater), Google Chrome 64 and greater, Mozilla Firefox 59 and greater, or Apple Safari 10.1/11.0/11.1 and greater.
  • Use the following instructions to complete the application and affidavit. NOTE: If all requested information is not provided on the application, you will not be able to submit your request online. You will need to either resubmit a fully completed application and copies of the front of your valid government photo IDs that contain a signature or make an appointment online to be issued the license in the Office.
    • Make photocopies of the front of your valid government photo ID (driver’s license, state ID, passport, etc.) and save them to your computer desktop. The ID must have a signature and match the signature on the affidavit; and, at least one of the valid government photo IDs must show proof of current Arizona residency.
    • Complete the online marriage license application form and then click on ‘Submit’ at the bottom of the application; names on the application must match exactly the IDs used to obtain the license.
    • Your application and ID’s have been submitted at this point and you may close the window.
    • You will receive an email from [email protected] to download the affidavit; only one affidavit signed by both applicants is needed
      • Print the affidavit
      • both applicants are to sign the affidavit with an original hand signature,
      • “save” it to your desktop and access the email you received and then “upload the signed affidavit.”
      • Click and drag the file to upload.
      • You may close the window at this time.
      • You will receive an email from [email protected] that your affidavit was received and you will soon be contacted by phone to obtain payment
  • Your application will be reviewed by staff and if accepted you will receive a phone call from staff within 5 business days, between normal business hours (8AM to 5PM, Mon thru Fri), at the phone number you listed on the application. NOTE: The call will be coming from a restricted phone number.
    • You will need to confirm social security numbers and dates of birth for both applicants.
    • You will need to make payment for the marriage license with a Visa, MasterCard or American Express card.
    • If there is a problem with the application or ID’s that you submitted your application will be rejected with an explanation as to why. You will need to start over and resubmit a new application, ID’s and affidavit.
  • Fees are $83.00 for the marriage license, $7 for postage and handling, and an additional $37.50 if you want to prepay for a certified copy of the license to be mailed back to you after the license is recorded.
  • After receiving payment, the license will be processed and mailed to the Arizona address within two business days via USPS First Class mail. A copy of your receipt will be included.

Arizona Marriage Certificate Requirements

Arizona Marriage Certificate Application Form PDF 2023 : Effective August 3, 2018, if you are under 16 years of age, you are prohibited from marrying.

If you are at least 16 years of age and under 18 years of age, the following are required:

  • either a certified copy of an Emancipation Order, OR a notarized Clerk’s Office parental consent form along with the front and back of your parent(s) or legal guardian’s identification, or have your parent(s) or legal guardian accompany you and present proper identification and sign the parental consent form in the presence of the clerk issuing your marriage license; and
  • a copy of your birth certification; and
  • one of the following government issued picture I.D.’s is required:
    1. current driver’s license;
    2. state or military I.D. card;
    3. current passport;
    4. or other governmental issued picture I.D
  • The younger applicant’s prospective spouse cannot be more than 3 years older than the younger applicant.

Arizona Marriage Certificate Instructions

Arizona Marriage Certificate Application Form PDF 2023 : Both parties must be present to obtain a marriage license. Each individual is required to provide a valid government-issued photo I.D., such as a driver’s license, passport, etc. to show proof of age and confirm identity.

Note: Effective May 28, 2021, a Matricula Consular I.D. card will be accepted as a valid form of identification for any type of marriage license to be issued, as long as the foreign government has used biometric identity verification techniques (i.e. fingerprint identification, retina scans, etc.) in issuing the consular identification card. Arizona Revised Statute 25-121(B) requires applicants to provide their social security number (SSN), if any. Except for child support purposes, the Clerk’s Office may not release SSNs without the applicant’s written request.

A copy of a divorce decree is not required; blood tests are not required.

  •  Complete the marriage license application form
  • A marriage license must be issued prior to the ceremony taking place, signed at completion of the ceremony, and returned by the officiator not more than 30 days after the ceremony has taken place in order for the license to be recorded.
  • You will receive your marriage license on the same day you apply for the license, and can be married on the same day, if you have made arrangements with an officiator of your choice to perform the ceremony. However, you have up to one (1) year from the date the marriage license was issued to get married, as the license expires one (1) year from the date of purchase.
  • After the bottom portion of the license has been returned and recorded, a certified copy can be requested and mailed to you for an additional $37.50 payable at the time you apply for the license or any time afterwards, or $30.50 if you provide a business-size, self-addressed stamped envelope at the time of your request.

Arizona Marriage License Fees

The fee for a marriage license is $83.00 payable by cash or money order, debit card or credit card. If you are purchasing a license at the Justice Courts, only money orders are accepted. After the bottom portion of the license has been returned and recorded, a certified copy can be requested and mailed to you for an additional $37.50 payable at the time you apply for the license or any time afterwards, or $30.50 if you provide a business-size, self-addressed stamped envelope at the time of your request. The fee to convert a traditional license to a covenant marriage is $30.00.

Covenant Marriage – Arizona Marriage Certificate Application

According to legislation (A.R.S. 25-901-906) which became effective Aug. 21, 1998, couples who are planning to apply for a Covenant Marriage License must provide a notarized statement from the clergy or marriage counselor who provided their premarital counseling. The notarized attestation/statement should confirm that:

  • the parties were advised about the nature and purpose of a covenant marriage;
  • the parties were advised regarding the limited reasons for ending marriage by legal separation or divorce; and
  • the parties were given a copy of the pamphlet, Covenant Marriages in Arizona, published by Arizona Office of the Courts (AOC).

At the Marriage License Office, the engaged couple will be asked to sign a marriage license affidavit and the “Declaration of Intent to Enter Into a Covenant Marriage” which states they met statutory requirements.

Requirements for Couples Who Want to Convert Their Marriage to a Covenant Marriage

Couples who are already married may convert their existing marriage to a covenant marriage. It does not matter where their license was issued. Solemnization (a wedding ceremony) is not required to accomplish the conversion to a covenant marriage.

The married couple will be required to sign a certificate of covenant marriage conversion and a “Declaration of Intent to Enter Into a Covenant Marriage Conversion” form at the Marriage License Office. The fee to convert a traditional license to a covenant marriage is $30.00. You can find more information about covenant marriages on the AZCourtHelp website.
